Overview

- The Jockey Club CADENZA Hub is one of the innovative community projects of CADENZA. It is a one-stop integrated health and social care centre managed by The Chinese University of Hong Kong and a team of multi-disciplinary health care professionals.
- The Hub adopts a user-focused case management approach to promote primary health and social care. It provides day care service and a wide spectrum of wellness, lifestyle modification, health maintenance courses, as well as on-site co-location services tailored for individual users. The Hub adopts a self-financing model.

Service Scope
| (1) Primary Health Services |
- Healthy lifestyle programmes: courses on managing physical and sensory changes associated with ageing, emotional management, positive thinking; health talks, physical activities, etc.
- Professional consultation, counselling, assessment and treatment services: individual counselling on lifestyle, diet, exercise, mental health; comprehensive geriatric assessment, balance and fall assessment, cognitive assessment; physiotherapy and occupational therapy, Traditional Chinese Medicine consultation and acupuncture.

- Chronic Disease Self-management Programme
- Diabetes, hypertension and dementia groups
- Skin care, incontinence care, medication management and memory enhancement programmes
- Caregiver support

| (3) Day Care Service |
- Personal care, cognitive training, physical rehabilitation training and interest groups
- Slow down physical and cognitive deterioration, accelerate rehabilitation process, improve activity level and self care ability

| (4) Other Services |
- Hearing assessment, consultation and hearing aids dispensing
- Rehabilitation aids and health products
- Coffee corner

Our Services
Experienced and multi-disciplinary professional team provides diversified services which include dementia day care, * Chinese medicine, occupational therapy, speech therapy, physiotherapy, podiatry, nutritional advice, counseling, medical referral and fitness instruction.
