Introduction:
Having chronic disease can be stressful for both the older people and their caregivers. “Chronic disease self-management model” helps people with chronic disease gain self-confidence in their ability to control their symptoms and manage how their health problems will affect their lives. Coping strategies and skills, such as problem-solving techniques, relaxation exercises, and communication skills are taught to empower people in managing their health. Thus, it is very important to build the self-confidence of the older people to manage their health condition actively, in order to promote their quality of life. This seminar aims to introduce the concept of self-care management and explore the effectiveness of chronic disease self-management model, so as to promote the self-efficacy in older people and lead a healthy lifestyle. Content:
- Overview the major chronic diseases in older people
- Introduce the concept of self-care management
- Chronic disease self-management includes 1) Positive attitudes and thinking, 2) Flexible problem solving skills, 3) Effective emotional management, 4) Regular relaxation exercise, 5) Healthy diet, 6) Knowledge of appropriate drug use, 7) Effective communication with health care professional and family
